I am currently offering tele-therapy through video and/or phone.
If you are ready to begin counseling, please contact me via email, text or phone call and we can schedule our initial appointment. If you are an existing client, you may also select "Make An Appointment" on the home page and choose a potential appointment time. I will confirm the time with you prior to the session.
Upon scheduling your session, you will receive a link to a client portal where you will enter your information. Forms include demographic information, insurance, and a general questionnaire. Completing the requested information prior to the session will help me understand how I can help you. If you are unable to access the information for any reason, please let me know. I can arrange to have the paperwork available prior to your session.
I am in network with Blue Cross Blue Shield PPO and Blue Choice.
If I am out-of-network for your insurance I can provide you with a monthly superbill/receipt upon request. Contact your insurance company if you have any questions regarding your insurance company's out-of-network benefits. Any reimbursement from your out-of-network insurance company will be sent to you.
Payment for all out-of-network clients is due at the time of service.
I accept cash, check and all major credit cards as forms of payment.
If you are unable to attend a session, please contact me as soon as possible. You may be charged a late cancellation fee for sessions cancelled less than 24 hours from scheduled counseling session time.
